Parent Letter to Schools for 2021-2022 School Year

Parents/guardians are welcome to submit this form provided by Awake IL for your student's file during the 2021-2022 school year. It cites statutes and protections regarding masking and Covid-19 vaccination. It will be updated here with any additional attachments or cited sources. Currently, the JAMA Assessment of Masking is added in the PDF download as an attachment.

Download, fill in student information, sign and mail to all applicable school district agents (superintendent, school principal, school nurse, and assigned teachers/staff). Email a photo of the signed copy to the above also ASAP.
